After reading research a few years ago stating that one of the best predictors of long term return to sport after ACL surgery was terminal knee extension, I started looking at it more. Not just for patients with post-op conditions, but for those with general knee pain. Many people concentrate on flexion, but for a joint to really be healthy and ready to accept load, full passive ROM/joint play should be restored in all motions.
After clearing the lower quarter of other common patterns, lumbar load on involved side, ankle dorsiflexion, tibial IR, etc, if knee pain still lingers, try evaluating and resetting terminal knee extension if there is a loss.
